Electronic Remittance Advice (ERA) is the ANSI X12 835 transaction that details how a payer adjudicated claims, including paid amounts, contractual adjustments (CARC/RARC), and patient responsibility—the digital equivalent of an EOB. ERAs include payment trace numbers that tie each remittance to its EFT or check. Example: an ERA on a $1,000 charge may show a CO-45 write-off of $300 and PR-2 coinsurance of $100, yielding a $600 payer payment.
Why it matters
For multi-location networks processing 50,000+ claims a month, ERAs are where denials and underpayments first show up. Reading them promptly can lower AR days by 2–4 while surfacing 1–3% underpayments that would otherwise be missed.
